How to Fill Teacher Identification Information Sheet (PINDICS FORM)

How to Fill PINDICS, Teacher Identification Information Sheet, Teacher Performance Scheet and Consolidation Sheet: We have known that National Curriculum Education Research and Training(NCERT) developed and prepared performance indicators for elementary school teachers. Performance indicators are called PINDICS, to be used to assess the performance and progress of teachers.


Each Performance Indicators are divided into 7 performance standards(PS) are the areas in which teachers perform their tasks and responsibilities. Under performance standards(PS) there are some specific tasks which are expected to be performed by teachers, these are termed as specific standards(SS). So, each perfomance standard has divided into specific standards(SS) and from also each specific standards are divided into 4 to 26 performance indicators(PI).

Each performance indicator is rated on four(4) rating point scale from 1 to 4 indicating the levels of performance.These can be used by only teachers themselves for assessing their own performance Quarter wise in a year and to make continuous efforts to reach the highest level. These can also used for teachers appraisal by supervisory staff or mentor to assess and to provide constructive feed back for improvement of teachers performance.

In this process assessment by Head Teacher or Cluster Resource Cnetre or Mandal Resource Centre also twice in a year, by using teacher’s selff assessment that is teacher identification information record, observation of actual classroom processes and dailogue with teacher, student and School Management members and it would be linked subsequently to Quality Monitoring Tools and consolidated at cluster, Mandal, District and State level.

In this subject Sarva Shiksh Abhiyan officials conducted trainings to the Head teachers at mandal level for understanding of PINDICS and pindics formats have been supplied to all school head teachers in the State. Aleready separate guidelines on pindics have been issued to the concerned district officials, mandal officers and school complex head masters.

  5.    PS 5. Professional Development
       SS: Self-study participation in in-service education programmes
       SS: Engagement in innovation and research
  6.    PS 6. School Development
       SS: Contributes to the organisation of school activities
  7.    PS 7. Teacher Attendance
       SS: Regularity and Punctuality

Each Specific Standards(SS) are divided into 4 to 26 Performance indicators(PI), these are available in Teacher Identification Information

  • 2. Approaching the expected standard
  • 3. Approached the expected standard
  • 4. Beyond the expected standard


Step I: Teacher Identification Information
Fill the following information like School Address, DISE Code No, State/UT, District, Block, Cluster, Name of the Teacher, Academic Qualification with Subjects: Senior Secondary : ; Graduation: Post-Graduation: ; Professional Qualification: , Any Other; Teaching Experience (in yrs.):; Teaching: Class (es):; Subject(s):; In-service Education Programmes attended during last five years…: LEP, Subject Trainings, CCE; and Achievements (Awards/ Distinctions) if any:

Step II: Writing the Rating Points
PS 1: Designing Learning Experiences for Children
SS: Planning for designing learning experiences.

Step III: Consolidated Rating of  Each Primary Standards
Total Score of indicators = (1+4+3+1 = 9)
Consolidated Rating of standard = Total Score / no.of indicators
= 9/4 = 2.25→ 2

Step IV: Grand Total of average score of 1 to 7 Standards
PS 1 + PS 2 + PS 3 + PS 4 + PS 5 + PS6 + PS 7
(2+3+2+2+2+2+2 = 15)
Overall Rating of all seven standards = Total Average Score / no.of indicators
15/7 = 2.14 → 2
The particular Teacher overall Performance: 2

Consolidated rating in each of the performance standard should be based on self assessment report of the teacher, classroom observation, and dialogue with teachers and students. It can be computed by dividing the total rating point under performance standard by total number of performance indicators approximated to the nearest whole number.

Overall Performance can be computed by dividing the grand total by 7 (total number of Performance Standards) approximated to the nearest whole number.

Each Teacher Performance Sheet to be completed at School Level by Head Teacher and after School Level Consolidation Sheet to be completed by School Head Master. Same Information can flow upwards level viz- Block, District or State, based on the information from Teacher Assessment Sheet–By Nodal Head Teacher/CRCC
